We are recruiting for a Strategic Change Manager for the Royal Borough of Greenwich. The Postholder will be accountable to the Integrated Director of Commissioning (Adults).
The post holder will be responsible/accountable for:
- Assisting the Integrated Director of Commissioning in the implementation of our integrated commissioning approach for adults/children across the Council and the NHS, which enables strong partnerships and high-quality services that support Greenwich residents to lead healthy, fulfilling and independent lives.
- Supporting senior leaders and staff through: - Partnership and co-production - Continued design and implementation of our integrated commissioning framework - Incorporating appropriate governance structures which support integrated, outcome focused approaches to health and care. - Working collaboratively with the market and residents - Leading staff across the system to implement transformational change including further integration of health, care and other forms of support - Overseeing the delivery of approaches to commissioning that address personalisation and whole person pathways.
- The post holder will also be aligned to one or more strategic change programmes which aim to trailblaze the new integrated commissioning approach. These will be transformational in nature and provide the opportunity for the post holder to bring their passion and enthusiasm for improving both outcomes for residents, improve and modernise working environments for practitioners and ensure best value and impact from our health and care community-based services.
- One of the change programmes for the post holder to play a key role in is the development and delivery of a new integrated Assistive Technology Enabled Care (ATEC) service. This is in partnership with our sector leading Digital and Customer Service team in Royal Borough of Greenwich. This is the first integrated ATEC model of its kind to be implemented which enables many opportunities for the post holder to bring existing skills and knowledge as well as to learn new and ground breaking ways of modernising health and care.
